What Is Charge Capture?
Charge capture is the process of recording every billable service, procedure, drug, and supply a patient receives so it can be coded and billed. When a service is delivered but never recorded as a charge, the organization is never paid for it, which is known as charge leakage.
How charge capture works
In most practices, clinicians document care in the EHR, and charges are created from that documentation, either automatically or by staff. The charge carries the procedure code, units, diagnosis, provider, date, and place of service, and it flows to the billing system to become a claim.
Where charges are missed
Charge leakage tends to happen where care crosses settings or teams, or where services are delivered in quantities that are easy to miscount.
- Hospital, facility, and after-hours services documented outside the office system.
- Drugs and supplies used during a visit but not recorded with correct units.
- Add-on services, such as extra time or additional procedures, not coded.

Charge lag and why it matters
Charge lag is the time between the date of service and the date the charge is entered. Longer lag delays payment and increases the risk of missing timely filing deadlines. Tracking lag by provider and location shows where documentation or workflow bottlenecks exist.
Charge validation
Capturing a charge is not enough; it also needs to be correct. Charge validation checks each charge against documentation and payer rules before it becomes a claim: whether units match the medication record, whether modifiers are required, and whether the diagnosis supports the service. Catching errors here is cheaper than working a denial.
