Chiropractic Accounting Software
Books that keep each clinic location, each chiropractor, prepaid care plans and personal injury receivables in their proper place.
Chiropractic Accounting Software carries what a chiropractic group bills and collects into its general ledger by location and provider. It books prepaid care plans as unearned until the visits are used. It also separates personal injury receivables from routine AR, and it ties every deposit back to the payments posted that day.

Accounting in a Chiropractic Practice
Many chiropractic groups are small but spread out: a few clinics, a handful of doctors, and sometimes a separate tax ID for each site. Accounting for that means more than one bank deposit a day. Unlimited Systems posts payments, adjustments and write-offs as balanced entries, tagged by location, provider and payer type. Revenue is recognized from remittance detail, not from what was charged. Card batches, EFTs, front desk cash and paper checks from auto carriers are each matched to their deposit. Prepaid care plans sit as a liability and move to revenue as each visit is delivered. Refunds owed to Medicare patients, after a payer pays a visit the patient already paid, show as credits waiting to go out. At month end, the entries export as standard journal registers for the practice's accounting system. The owner then sees margin by clinic, not one blended number.
Why Is Accounting Different in Chiropractic?
Chiropractic groups often grow one clinic at a time. Each new site adds a front desk, a deposit, and sometimes its own tax ID. Owners want to know whether each clinic pays for itself, and whether each associate doctor brings in what they cost. That needs accounting by location and by rendering provider, fed straight from billing instead of rebuilt from bank statements.
Cash-pay care plans create an accounting question many offices miss. When a patient prepays for a block of visits, the cash is in the bank, but the visits have not happened yet. Until they do, the money is owed back in care or in a refund. Booking prepaid plans as revenue on the day they are sold inflates that month and leaves nothing for the months when the visits occur.
Personal injury balances are a third issue. Auto and liability cases can stay open for a long time, and settlements may pay less than the full bill. Mixing those balances into routine AR makes the books look healthier than they are. Accounting for chiropractic keeps them in their own bucket, so owners and their accountants can judge how much of that money will really arrive.
What Chiropractic Accounting Software Handles
- Prepaid care plans under ASC 606
- Payment received before the service is delivered is a contract liability until the visits are provided. Each visit used moves its share of the plan from liability to revenue in the ledger. (FASB Accounting Standards Update 2014-09: Revenue from Contracts with Customers (Topic 606))
- Refunds owed after ABN collections
- When Medicare or a secondary insurer pays a visit the patient already paid under an ABN, the practice must refund the patient. Those amounts are carried as credits owed, not as income. (CMS Medicare Claims Processing Manual, Chapter 30, Section 50.15 (ABN collection of funds and refunds))
- Usual fees for ABN-covered care
- Funds collected under a signed ABN are not limited to the Medicare allowed amount, and the practice may charge its usual fee. Maintenance visit revenue is booked at the price the patient agreed to. (CMS Medicare Claims Processing Manual, Chapter 30, Section 50.15 (ABN collection of funds and refunds))
- Reporting and returning overpayments
- Medicare overpayments that the practice identifies must be reported and returned under federal rule. Overpayments are tracked as liabilities from the day they are found until they are repaid. (42 CFR 401.305: Requirements for reporting and returning of overpayments)
- DMEPOS supplies sold in the office
- A chiropractor can bill Medicare for supplies such as braces only with a valid DMEPOS supplier number. Supply sales are kept on their own revenue account, so their margin is visible apart from visit revenue. (CMS Medicare Claims Processing Manual, Chapter 12, Section 220 (Chiropractic Services))
How Does Chiropractic Accounting Software Work?
1. Post with location and provider
Every payment, adjustment and write-off is tagged with the clinic, the rendering chiropractor and the payer type. Those tags become cost centers in the ledger.
2. Match each deposit
Card batches, EFTs, cash and paper checks are matched to the payments posted that day. Any gap shows up as unapplied cash to clear.
3. Defer prepaid plans
Prepaid care plans post to a liability account when sold. Revenue is recognized visit by visit as the plan is used.
4. Hold injury receivables apart
Auto, liability and workers' compensation balances are tracked on their own receivable account. Settlement reductions post as their own adjustment type.
5. Track credits and refunds
Patient overpayments and amounts owed back to payers stay as credits until they are paid out. They are never netted silently against new charges.
6. Export for the close
At month end, entries export as standard journal registers to the practice's accounting system or ERP. The close starts from figures billing already agrees with.
Chiropractic Accounting Software vs. Generic Software
| Area | Generic approach | Unlimited Systems |
|---|---|---|
| Locations | One ledger for every clinic, split by hand later. | Entries tagged by location from the moment they post. |
| Associate doctors | Collections per doctor rebuilt from reports. | Collections tied to the rendering provider on each line. |
| Prepaid care plans | Booked as revenue when sold. | Held as a liability and earned visit by visit. |
| Injury receivables | Mixed into routine AR. | Carried on their own account with settlement adjustments. |
| Month-end close | Deposits and charges re-keyed into the books. | Journal registers exported from posted activity. |
Who Uses It in a Chiropractic Practice
- Clinic owner: Margin by clinic and by doctor, so decisions about a new site or a new associate rest on real numbers from chiropractic analytics.
- Office manager: A daily deposit that matches the day's posted payments, with any gap shown before the bank statement arrives.
- Outside accountant: Clean journal registers by location, with prepaid plans deferred and injury receivables kept separate.
Chiropractic Accounting Software FAQs
Accounting treats a prepaid care plan as money owed in care until the visits happen. The payment posts to a liability account when the plan is sold. As each visit is delivered, its share moves to revenue. If the patient stops early, the unused part is either refunded or handled under the plan's terms, and the ledger shows which applies.
Each payment, adjustment and write-off carries the clinic it belongs to, so each location acts as its own cost center. Groups with a separate tax ID per site can keep those entities apart in one database. Owners can view a single clinic or roll every location up, and the month-end export keeps the same split for the outside accountant.
Personal injury balances can stay open for a long time, and settlements may pay less than the full charge. Keeping them on their own receivable account stops them from making routine AR look larger than the cash it will produce. Settlement reductions post as their own adjustment type, so owners can see how much injury work really collects.
When Medicare or a secondary payer pays for a visit the patient already paid, the patient is owed a refund. That amount is carried as a credit owed, not as income. Overpayments the practice finds from Medicare itself are tracked as liabilities until they are returned. Both stay visible until they are paid out.
Posted activity exports as standard general ledger journal registers that sync with mainstream accounting and ERP systems, including QuickBooks and NetSuite. Entries carry their location, provider and payer tags. The practice's accountant works from the same figures the billing team posted, without re-keying deposits or charges.
Every posted line carries the rendering chiropractor, so collections, adjustments and write-offs can be totaled by doctor. That gives owners a fair view of each associate's production. It also supports pay plans based on collections, because the numbers come from posted payments rather than from scheduled visits or billed charges.
