Clinical Research Interoperability Software

Exchange study data with payers, hospitals, labs, registries and sponsor systems over X12, HL7, FHIR and APIs, with the trial number and study link kept intact at every hop.

Clinical Research Interoperability Software moves study data between the research site and everyone outside it: payers, hospitals, labs, registries and the systems sponsors and the site use to run trials. Claims carry the trial number in the standard X12 field, study records can follow FHIR research resources, and each exchange keeps its link to the study.

Interoperability in a Clinical Research Practice

A research site trades data with more outside parties than most practices. Payers need claims with trial fields and send back remittances and status. A hospital partner may run the infusions or scans for a study and bill its own share. Labs may split work between local tests billed to insurance and central tests the sponsor pays for. Registries tied to Coverage with Evidence Development expect the patient's data. The site's own trial management tools hold the study calendar and enrollment. Unlimited Systems connects through the channel each party supports: X12 transactions through the clearinghouse, HL7 messages, FHIR resources and APIs. Trial claims carry the 8-digit number in Loop 2300 REF with qualifier P4. Study and enrollment data can be shaped to the FHIR ResearchStudy and ResearchSubject resources where a partner uses them. Data sent to a sponsor is limited to what the participant's research authorization covers, and each interface is monitored as it runs.

Why Is Interoperability Different in Clinical Research?

Interoperability at a research site has a third party most practices don't have: the sponsor. Sponsors and CROs want enrollment updates, visit completion and invoice detail, and they have their own systems for each. Research staff often key the same visit into the EHR, a trial management tool and a sponsor portal. Every copy is a chance for the visit counts to disagree, which shows up later as an invoice dispute.

The payer side has its own research fields. Medicare checks that each trial claim carries a valid trial number, and contractors confirm it against CMS's list of approved trials and registries. A claim that loses the number between systems comes back unprocessable. Standard X12 placement, kept the same from the charge to the clearinghouse, prevents that loss.

Sharing also has limits. A sponsor is entitled to what the participant agreed to share in the research authorization, not to the full chart. Hospital partners and labs need enough to bill their own part, not every detail of the protocol. Interoperability for research has to move data quickly to each party while sending each one only what it should receive.

What Clinical Research Interoperability Software Handles

Trial number on X12 claims
Medicare wants the 8-digit trial number on the 837 in Loop 2300 REF02 with REF01 set to P4, for both professional and institutional claims. The number is carried from setup to the X12 claim without being retyped. (CMS Medicare Claims Processing Manual, Chapter 32, Sections 69.2 and 69.6: Clinical Trial Billing Requirements)
Trial validity checks
Medicare contractors verify a trial, study or registry by checking CMS's clinical trials and registry website. Sites can check new studies against the same list during setup, before the first claim goes out. (CMS Medicare Claims Processing Manual, Chapter 32, Sections 69.2 and 69.6: Clinical Trial Billing Requirements)
FHIR ResearchStudy
FHIR's ResearchStudy resource holds study identifiers, title, status, phase, sponsor, principal investigator and sites. Study data can be mapped to it when a partner system uses FHIR for research. (HL7 FHIR R4: ResearchStudy resource)
FHIR ResearchSubject
ResearchSubject links a patient to a ResearchStudy, with a status, a participation period, a consent reference and the assigned arm. Enrollment exchanged in this form keeps the patient and study tied together. (HL7 FHIR R4: ResearchSubject resource)
Disclosures to sponsors
A HIPAA research authorization may be combined with the study consent and may run to the end of the research study. Data sent to a sponsor is limited to what that authorization covers. (45 CFR 164.508: Uses and disclosures for which an authorization is required)
CED registries
Under Coverage with Evidence Development, CMS covers some items only within an approved study or registry. Services billed under a CED decision carry the study number, linking the claim to the registry the site reports to. (CMS: Coverage with Evidence Development)

How Does Clinical Research Interoperability Software Work?

  1. 1. Map each outside party

Payers, hospital partners, labs, registries and sponsor systems are each set up with the channel they support. Some use X12, some HL7, and some FHIR or an API.

  2. 2. Keep the study link on every message

Each claim, result and record carries the study and participant link from the start. The trial number reaches the clearinghouse in the right field without being retyped.

  3. 3. Extend interoperability past the EHR

Clinical data from the site's own EHR arrives through research EHR integration. Outside results and partner claims come in through the same integration layer.

  4. 4. Limit what each party receives

Sponsor-bound data is checked against the participant's research authorization. Hospital and lab partners get what they need to bill their share, and no more.

  5. 5. Monitor every interface

Each connection is monitored while it is running. When a feed from a lab, payer or partner stops, staff see it before study charges or results go missing.

Who Uses It in a Clinical Research Practice

  • IT manager: One interoperability layer for payers, hospitals, labs, registries and sponsor systems, each on the standard it supports.
  • Research operations director: Less double entry between the EHR, trial tools and sponsor portals, so visit counts agree everywhere.
  • Privacy officer: Outbound research data checked against each participant's authorization before it leaves the site.

Clinical Research Interoperability Software FAQs

Interoperability for a research site means exchanging study data with payers, hospital partners, labs, registries and sponsor systems through standards each one supports. Claims use X12 with the trial number in its set field. Clinical messages use HL7. Study and enrollment records can use FHIR research resources or an API. Each exchange keeps the study and participant link intact.
