Clinical Research Platform System Design
A cloud platform whose data model knows studies, protocol versions, enrollments and sponsors, keeps every change, and runs research and patient care as linked entities in one database.
Clinical Research Platform System Design is the architecture under a research site's billing: a cloud platform with a data model that treats each study, protocol version, enrollment and sponsor as real records. Every change is kept, research can run as its own entity beside the practice, and access follows each person's role and studies.

Platform System Design in a Clinical Research Practice
Most billing systems know patients, payers and claims. A research site also needs the system to know studies. In Unlimited Systems, a study record holds its protocol number, sponsor, trial number, qualifying basis and each approved version of its coverage analysis. An enrollment links a participant to a study with dates and status. Visits, charges, claims and sponsor invoices all point back to the study and to the grid version in force on the date of service. Because the platform records every data change instead of overwriting it, a visit billed under an old grid can still be explained after an amendment. A research division with its own Tax ID runs as a separate entity in the same database, so shared patients stay linked while the books stay apart. The cloud platform scales with volume, so a study close-out or a big sponsor audit doesn't slow daily billing. Access rules follow the organization chart, so staff see only the studies they work on.
Why Is Platform System Design Different in Clinical Research?
Platform System Design matters more in research because the facts change while the study runs. Protocol amendments add visits, drop procedures and move services between payers. A system that stores only the current setup can't say how a visit should have been billed when it happened. Keeping each version of the grid, and tying each charge to the version in force, is what lets a site defend its billing later.
Research sites are also more than one business. A practice may run studies under the same Tax ID, a separate research company, or a mix across locations. The same patient can be a routine patient of the practice and a participant in a study. The data model has to keep shared patients linked across those entities while each one keeps its own ledger, reports and security.
Audits in research reach back. Sponsors audit visits from early in a study, monitors check source records, and Medicare's overpayment rule has a long lookback. A platform that overwrites history makes those questions hard to answer. One that records every change, and logs who viewed or edited what, gives the site a complete trail without a separate archive.
What Clinical Research Platform System Design Handles
- Study identifiers as structured data
- CMS requires the trial name, sponsor and sponsor-assigned protocol number in the participant's record, and an 8-digit NCT number on claims. The study record stores each as its own field, so claims and records pull from one source. (CMS Medicare Claims Processing Manual, Chapter 32, Sections 69.2 and 69.3: Clinical Trial Payment and Records)
- Change history for overpayment lookback
- Medicare's overpayment rule applies to overpayments identified within a set lookback period after receipt. Because every data change is kept, the site can rebuild how any past visit was billed across that period. (42 CFR 401.305: Requirements for reporting and returning overpayments)
- Audit controls
- The HIPAA Security Rule requires mechanisms that record and examine activity in systems holding electronic PHI. Views and edits of participant accounts are logged, which also helps when a monitor or sponsor reviews records. (45 CFR 164.312: Technical safeguards, including audit controls)
- Authorization terms on the enrollment
- A HIPAA research authorization may name the end of the research study as its expiration. The enrollment record stores the authorization and its end point, so outbound data checks have something to test against. (45 CFR 164.508: Uses and disclosures for which an authorization is required)
- Study records shaped for exchange
- FHIR's ResearchStudy resource holds identifiers, status, phase, sponsor, principal investigator and sites. Keeping study records in a similar shape makes mapping to FHIR partners simpler. (HL7 FHIR R4: ResearchStudy resource)
- Payer logic by study type
- For Medicare Advantage enrollees, Original Medicare pays routine costs in qualifying trials, while plans pay for IDE studies and most CED studies. The study record stores its type, so payer routing follows it. (CMS Medicare Managed Care Manual, Chapter 4, Section 10.7: Clinical Trials)
How Does Clinical Research Platform System Design Work?
1. Model the study
Each study is created with its protocol number, sponsor, trial number, qualifying basis and budget. Its coverage analysis is loaded as a dated version that later amendments add to rather than replace.
2. Link enrollment and visits
An enrollment ties a participant to the study with dates, status and authorization terms. Visits, charges and claims point to both the enrollment and the grid version in force.
3. Separate entities, one database
Research under its own Tax ID runs as its own entity. Shared patients stay linked, and each entity keeps its own study-level books and access rules.
4. Control and log access
Roles decide which studies and fields each user sees. Activity is logged to support HIPAA audit controls and sponsor monitoring visits.
5. Scale and connect
The cloud platform scales with transaction volume and storage. Data moves to partners through the channels covered under research interoperability, and out to leaders as study reports.
Clinical Research Platform System Design vs. Generic Software
| Area | Generic approach | Unlimited Systems |
|---|---|---|
| Study data | Kept in spreadsheets beside the billing system. | Studies, versions, enrollments and sponsors stored as real records. |
| Amendments | New setup overwrites the old one. | Each grid version kept and tied to the visits billed under it. |
| Research entities | A second system for the research company. | Separate entities in one database, with shared patients linked. |
| Audit trail | Rebuilt from memory and email. | Every data change recorded, with access logged. |
| Scale | Slows when close-out or an audit adds load. | Cloud capacity that grows with volume. |
Who Uses It in a Clinical Research Practice
- IT manager: A cloud platform with a study-aware data model, role-based access and activity logs, without servers to run.
- Compliance officer: A complete change history for any visit, claim or grid version, ready for a sponsor audit or a Medicare review.
- Chief executive: Room to add studies, sites and a research entity without adding systems, with one view across all of them.
Clinical Research Platform System Design FAQs
Platform system design for a research site is the architecture under its billing. It covers a data model that knows studies, protocol versions, enrollments and sponsors, a history that keeps every change, entity structure for research run under its own Tax ID, role-based access, and cloud capacity. Together those let research and patient care run on one platform without mixing their books.
Each approved version of a study's coverage analysis is stored as its own dated version. Visits and charges point to the version in force on the date of service. When an amendment arrives, a new version is added and the old one is kept, so past visits can still be explained exactly as they were billed.
A research company with its own Tax ID can share the platform as a separate entity in the same database. Its ledger, reports and access rules stay its own, while patients shared with the practice stay linked. Leaders can review the research entity alone or roll it up with the practice, without moving data between two systems.
The platform records every data change rather than overwriting it, and logs who viewed or edited each account. A sponsor audit, a monitor's records review or a Medicare question about an old claim can be answered from that history. The HIPAA Security Rule also expects audit controls on systems holding patient data, which these logs support.
Study data is visible according to each person's role and studies. A coordinator sees the participants and charges for their own studies, a research biller sees research claims across studies, and finance sees study ledgers. Access follows the organization's structure, so adding a new study or entity doesn't mean rebuilding security by hand.
Study close-out, sponsor audits and month-end can all add load at once. The cloud platform scales with transaction volume and storage needs, so reconciling a large study or running a long history report doesn't slow posting and billing for everyone else. Capacity grows with the site's study count instead of being sized for an average day.
