Medical Billing in a Orthopedics Practice

Orthopedic claims look different depending on who sends them and who receives them. A surgeon's bilateral knee scope goes to Medicare on one line with modifier 50 and one unit. The same case billed by an ASC is reported on two lines, one with RT and one with LT. A viscosupplement injection needs the drug code, the right number of units and, for some products, the NDC. A brace claim goes to a DME contractor, not the usual Medicare contractor. A work injury claim needs the injury date, the accident boxes and the carrier's claim number. Unlimited Systems builds each claim in the format its payer expects and scrubs it at checkout. It then sends it as an 837 through a real-time clearinghouse link. Acknowledgments and claim status come back to the same screen. Rejections are fixed and resent quickly, and secondary claims file on their own once the primary pays.

Why Is Medical Billing Different in Orthopedics?

Most of the hard work in orthopedic billing happens on the claim form. Modifiers carry a lot of weight: RT and LT, 50 for bilateral, 51 for added procedures, 24, 58, 78 and 79 inside the global period, finger and toe modifiers, and 80, 82 or AS for assistants. A claim that pairs the wrong modifier with a code is often denied as inconsistent. Reporting rules also differ by setting. Medicare wants surgeons to report bilateral cases with modifier 50 on one line, while ASCs report them on two lines with RT and LT.

Orthopedic practices also send claims to more kinds of payers. Health plans and Medicare take standard electronic claims. Work injury carriers often want the claim number, the injury date and supporting notes. Auto and no-fault carriers have their own forms and limits. Braces dispensed in the office are billed to a DME contractor under the practice's DMEPOS supplier number. The CMS-1500 boxes for work, auto and other accidents tell each payer which one should pay first.

Tracking matters as much as sending. A surgical claim stuck in a clearinghouse queue holds back a large payment. Orthopedics medical billing software watches the 277CA acknowledgment and claim status for every claim, and routes rejections to a person right away. Rule checks before the claim leaves are handled by orthopedic charge validation, and the wider view of cash and metrics belongs to orthopedic RCM.

What Orthopedics Medical Billing Software Handles

Bilateral reporting: surgeon versus ASC
Surgeon claims for bilateral procedures go on one line with modifier 50 and one unit, unless the code is already bilateral. ASC claims for the same case go on two lines with RT and LT. Claims are built to match the billing entity. (Medicare NCCI Policy Manual 2026, Chapter IV: Musculoskeletal System (CPT 20000-29999))
Assistant at surgery modifiers 80, 82 and AS
Assistant claims are built with 80 for an assistant surgeon, 82 when no qualified resident was available, or AS for a PA or NP. The claim is linked to the primary surgeon's claim for the same case. (CMS MLN907166 Global Surgery Booklet (December 2025))
NDC reporting for hyaluronan codes
For codes such as J7322, the NDC is reported in loop 2410 with the N4 qualifier on electronic claims, or in item 19 on paper. The claim builder adds it from the drug record. (WPS Billing and Coding Article A56157: Intraarticular Knee Injections of Hyaluronan)
JW and JZ drug modifiers
Under Medicare Part B, separately paid drugs from single-dose containers need JW for discarded drug on its own line, or JZ when none was discarded. The claim builder adds the right one from the dose record. (CMS Discarded Drugs (JW and JZ modifiers))
Accident and injury items on the CMS-1500
Items 10a to 10c, the accident state and item 14 are filled from the case for every work or accident claim. A yes in item 10 signals that another payer may be primary to Medicare. (Medicare Claims Processing Manual, Chapter 26: Completing and Processing Form CMS-1500)
Surgeon claims at an ASC (POS 24)
When the surgeon operates at an ASC, the professional claim carries POS 24. The ASC sends its own facility claim. Each claim is built and tracked on its own. (CMS Place of Service Code Set)

How Does Orthopedics Medical Billing Software Work?

  1. 1. Build the claim

Validated charges are turned into a claim in the payer's format. Codes, modifiers, units, diagnosis pointers and NDCs are placed on the right lines. Work and accident cases add the injury date, claim number and accident items.

  2. 2. Scrub before it leaves

The claim is checked against payer edits, NCCI rules and coverage limits at checkout. Errors are fixed while the details are fresh. Clean claims move on.

  3. 3. Send through the clearinghouse

Claims go out as 837 files over a real-time clearinghouse link. Brace claims go to the DME contractor, and work injury claims go to the carrier. Paper or attachments are sent where a carrier requires them.

  4. 4. Watch acknowledgments and status

999 and 277CA responses show whether each claim was accepted. Status checks follow the claim after that. Rejections go to a queue with the reason.

  5. 5. File secondary and close out

When the primary payer pays, the secondary claim is built and sent. For Medicare patients with a work or accident primary, the primary's payment details go with it. The claim closes when it is paid or moves to denial work.

Who Uses It in a Orthopedics Practice

  • Billing specialist: Claims built in the right format for each payer, with modifiers, units and NDCs already placed.
  • Billing manager: A single view of claim status across health plans, Medicare, work injury carriers and the DME contractor.
  • ASC business office: Facility claims built with RT and LT lines for bilateral cases, apart from the surgeon's claims.
  • Surgeon: Surgical claims that go out right the first time, so large payments are not held up.
